Workplace Health Award The Eastern Ontario Health Unit is proud to offer organizations the opportunity to apply for its Workplace Health Award. The Workplace Health Award recognizes workplaces that encourage their employees to develop and maintain a healthy lifestyle. Your contributions make a difference in the lives of your employees and their families. This award is open to all workplaces within the United Counties of Prescott-Russell, the United Counties of Stormont, Dundas & Glengarry, and the City of Cornwall. It will allow your organization to be recognized within the community. The 2012 awards will take into consideration the period of June 1, 2012 to June 1, 2013. Applications are due no later than August 30, 2013 at 4:30 p.m. The Workplace Health Award committee will evaluate all entries and tabulate the scores. The Award recipients will be notified this fall. Awards will be presented during the Eastern Ontario Health Unit’s Workplace Health Promotion Workshop. Review the Workplace Health Award Application Form, check off the topics that your organization has addressed, and add any others that are not listed. Once completed, you can submit your application in three ways: By email Scan the completed Workplace Health Award Application and email it to scharbonneau@eohu.ca.
