School-Based Immunization Clinics (Grade 7)

Each year, school-based clinics are held for students in Grade 7, to provide immunization against the three following diseases:

  • Meningococcal disease (A, C, Y, W-135)
  • Hepatitis B
  • Human Papillomavirus (HPV-9)

The meningococcal vaccine, which protects against meningitis, is mandatory to attend school in Ontario, as per the Immunization of School Pupils Act. The hepatitis B and HPV vaccines are not mandatory to attend school, but are strongly recommended for all boys and girls in grade 7 as they can protect against serious illnesses, including liver disease and some types of cancer. Vaccines are administered at school by a public health nurse from the EOHU. Parents and legal guardians of grade 7 students will receive a consent form and information about the vaccines via the student’s school.

To ensure the student in your care can be immunized at the upcoming school clinics, you’ll need to complete the school-based vaccination consent form (also available below). If you are unable to complete the form electronically, call the health unit.

School-Based Vaccination Consent Form

School Immunization Clinics - Vaccine Information

To ensure the student’s consent is received prior to the school clinic date, parents and legal guardians must complete and submit the form within two weeks of receiving a letter from the student’s school. The completed consent form must be submitted regardless of the student being vaccinated or not.

If the student has already been vaccinated, parents and legal guardians must submit the student’s immunization record online or call the EOHU Immunization Department at 1-866-318-0522 to update their immunization records. Visit the Report a Vaccine to the EOHU webpage to learn more about how to submit a student’s immunization record.

On the day of the clinic, students should make sure to eat breakfast which can prevent them from feeling dizzy or unwell during their immunization. Students should also wear a short-sleeved t-shirt, as the vaccines will be given in the upper arm.

If you have any questions, call the EOHU at 613-933-1375 or 1-800-267-7120.

The clinic schedule may change on short notice. Please contact your child’s school for the most up to date information.
